What is Glyxambi?

Glyxambi is a medicine that falls into the categories of oral hypoglycemic drugs. It is a combination drug because it contains two main ingredients: linagliptin and empagliflozin. The mechanism of action of empagliflozin, independent of insulin metabolism and the functional state of pancreatic beta cells, differs from and complements the mechanisms of action of drugs currently used to treat type 2 diabetes mellitus. Empagliflozin has been found to potentiate the action of all hypoglycemic drugs, even those with different mechanisms of action, including DPP-4 inhibitors. Long-term use of the combination of empagliflozin plus linagliptin led to a significant increase in patients’ sensitivity to insulin compared with the use of linagliptin and empagliflozin alone.

Linagliptin is an inhibitor of the DPP-4 enzyme, which is involved in the inactivation of the incretin hormones glucagon-like peptide type 1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). Linagliptin actively binds to the DPP-4 enzyme (the relationship is reversible), which causes a steady increase in the concentration of incretins and long-term preservation of their activity. This substance enhances the biosynthesis of insulin and its secretion by beta cells of the pancreas at normal or elevated blood glucose concentrations. In addition, it helps reduce glucose production in the liver. Linagliptin increases glucose-dependent insulin secretion and reduces glucagon secretion, which leads to normalization of blood glucose concentrations. Empagliflozin is a reversible, highly active, selective and competitive inhibitor of sodium glucose transporter type 2. Empagliflozin improves glycemic control in patients with type 2 diabetes mellitus by reducing renal glucose reabsorption.

Glyxambi Side Effects

The most common side effect of Glyxambi is urinary tract infection. In general, the safety profile of this combination of active ingredients, linagliptin and empagliflozin, is comparable to the safety profile of linagliptin and empagliflozin used separately. Those adverse reactions that are caused by taking Glixambi are observed with monotherapy with both linoleptin and empagliflozin. No additional side effects were observed when using a combination of these active ingredients compared to monotherapy with each of them. Common side effects include hypersensitivity reactions, hypoglycemia (especially when used in combination with sulfonylureas or insulin), cough, nasopharyngitis, urinary tract infections, frequent urination, vaginal candidiasis, vulvovaginitis, balanitis and other genital infections. In addition, patients often experience itching. Less common side effects include urticaria, angioedema, hypovolemia, pancreatitis, dysuria, and rash.

Glyxambi Uses

Glyxambi is indicated for use in patients with type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control. It is noteworthy that if recommendations regarding physical activity and diet are neglected, the medicine rarely helps to achieve any tangible result. This medication is usually not recommended for patients at risk of developing hypovolemia. These groups of patients include primarily those taking antihypertensive medications and people with a history of arterial hypotension. Also, restrictions on use include diseases of the gastrointestinal tract leading to fluid loss. Glyxambi is not recommended for persons over 75 years of age, or in combination with sulfonylurea derivatives or insulin. Patients with chronic genitourinary tract infections should also avoid this drug. Conditions such as low secretory activity of pancreatic beta cells and a history of diabetic ketoacidosis, as well as a diet low in carbohydrates, are restrictions on the use of this medicine.

Glyxambi Weight Loss

Empagliflozin improves glycemic control in patients with type 2 diabetes mellitus by reducing renal glucose reabsorption. By inhibiting the reabsorption of glucose in the kidneys, it leads to an increase in its excretion by the kidneys, which causes a decrease in the concentration of glucose in the blood, both after a single oral dose and with long-term use. In addition, renal excretion of glucose causes calorie loss and leads to weight loss. In clinical studies in patients with newly diagnosed type 2 diabetes mellitus, it was proven that the use of Glyxambi leads to a statistically significant decrease in glycosylated hemoglobin (HbA1c), a decrease in fasting plasma glucose concentration, as well as a decrease in blood pressure and body weight.

Glyxambi Dosage

Glyxambi is available in two dosages. In both cases, these are film-coated tablets. In the first case, each tablet contains 5 mg of linagliptin and 10 mg of empagliflozin, in the second case – 5 mg of linagliptin and 25 mg of empagliflozin.

The method of use and dosage regimen of Glyxambi for each patient is determined by the doctor. The compliance of the dosage form of the drug used with the indications for use and dosage regimen should be strictly observed. Glyxambi is taken orally, regardless of food intake, at any time of the day (preferably at the same time every day).

The recommended starting dose of Glyxambi is 5 mg linagliptin + 10 mg empagliflozin once daily. If this dose is well tolerated by the patient, but doesn’t provide adequate glycemic control, it can be increased to 5 mg linagliptin + 25 mg empagliflozin once daily.

When using the drug in combination with insulin and sulfonylurea derivatives, it may be necessary to reduce the dose of insulin or sulfonylurea derivatives in order to reduce the risk of hypoglycemia.
